Part 2: Summer 2008 Leadership Institute Registration

Part 3: Please respond briefly to the following questions.

1. Describe your teaching experience. Address the following questions and anything else you want us to know about your experience:
a. How long have you been teaching mathematics and which math classes have you taught?

b. What do you enjoy about teaching mathematics? What is your greatest concern about teaching math?

c. Why did you begin teaching CPM?

d. How were the CPM workshops helpful? How would they have been more helpful?

e. What was the hardest part of your first year of teaching CPM? What helped you be successful during the year?

2. State why you want to be a teacher-leader for CPM, the strengths you would bring to the position, and note any experience you have working in a leadership or workshop format with adults.
